Registration InformationFALL 2009 REGISTRATION Registration is at the Burlingame Recreation Center, 850 Burlingame Avenue adjacent to Washington Park. In Person Early Registration Saturday April 4, 2009 from 9:00am - 1:00pm All boys and girls born between August 1, 1990 and January 31, 2005 are eligible. Early registration is $120.00 per child. After May 2nd, 2009 the normal registration cost is $160.00 per child. Non residents of Burlingame must pay $30.00 extra. Please note: Those that register after May 2nd will be put on a waiting list. Those on the waiting list will be placed on a team only if the number of trained, volunteer coaches and referees allows. We are a volunteer organization dependent on parents to volunteer. Without more volunteers our program must become more limited. Cash or Check Only! Check made payable to AYSO Region 63 only – NO Credit Cards This year you MUST pre-register online at http://www.eayso.com/ and bring 3 (THREE) copies of the registration form (all copies should be signed). Bring a birth certificate or other proof of birth date if your child is a new player in Burlingame. Coaches, Referees, and other Volunteers MUST also fill in a volunteer form online at http://www.eayso.com/ and bring 3 (THREE) copies of the volunteer form along with their drivers license (all copies should be signed). NO COACH will be signed up unless they have filled in the form online and bring the copies with them to registration! A limited number of computers will be available during registration for those who can not access the eAYSO website. OPENING WEEKEND 2009: Saturday and Sunday, September 12th & 13th |
