Registration Information

FALL 2008 REGISTRATION

Registration is at the Burlingame Recreation Center, 850 Burlingame Avenue adjacent to Washington Park.

The four regular registration dates for the Fall 2008 season have ended. If you still wish to register your child, there will be one and only one late registration date on Monday June 2nd from 6-8 pm in the computer room of the Burlingame Recreation Center.

Please note, however, that late fees will be assessed at this time. Those fees are: For Burlingame residents: $160.00 For non-residents: $190.00

Please also note that your child will be placed on a waiting list and will be placed on a team only if we have enough volunteer coaches and/or referees. Please consider, as a parent of a young player, signing up to be trained to become a referee for our organization, as this will guarantee that your child will be placed on a team for the Fall.

For the 2008 season all player registration forms MUST be completed online at the AYSO website http://www.eayso.org. Three copies of the completed form must be brought to registration with your payment (cash or checks made payable to AYSO Region 63 please) in order to complete the registration process. New players to Burlingame must also being a copy of a birth certificate to prove date of birth.

* Increase of 2008 fees due to increase in per player city fees, increased city lighting fees and a concerted effort by AYSO to raise money to improve and increase fields for youth sports in Burlingame.

This year you MUST pre-register online at http://www.eayso.com/ and bring 3 (THREE) copies of the registration form (all copies should be signed).

Bring a birth certificate or other proof of birth date if your child is a new player in Burlingame.

Coaches, Referees, and other Volunteers MUST also fill in a volunteer form online at http://www.eayso.com/ and bring 3 (THREE) copies of the volunteer form along with their drivers license (all copies should be signed). NO COACH will be signed up unless they have filled in the form online and bring the copies with them to registration!

A limited number of computers will be available during registration for those who can not access the eAYSO website.
